Personal Care Assessment Unit


services-assessmentThe Personal Care Assessment begins when the Medical Director or the individual’s personal physician completes a physical examination, medical history and a medication review.  A functional assessment determines the individual’s ability to perform the activities of daily living, and the in-house rehabilitation staff assesses stamina, strength, duration and balance, and the ability to ambulate safely.  Cognitive and memory testing also occur.  A staff member gathers information regarding dietary preferences and special dietary needs, and identifies each individual’s interests, skills and hobbies.  Any family members are invited to assist in identifying the person’s needs.  Windsor Gardens then shares all findings and recommends the appropriate care level for the incoming resident.
